Used to provide health history and medication information for your camper,  a signed hard copy of this completed form must be on file BEFORE your child can attend camp. This can be mailed to the Registrar/Camp Director.
Day camp staff cannot distribute medications to campers, whether over-the-counter or prescription, without parent or guardian permission. In order for the day camp health supervisor to give any medication brought from home to campers, a signed copy of this completed form must be on file.
Used to indicate who is allowed to pick up your camper from day camp.
All campers and parents are asked to sign this Behavior Agreement for Day Camp.
ONLY fill out if using Mail-in Registration.

$25 Late registration fee starts.

Registration closes (or when we get to capacity)

June 22nd
Leader Training! New leaders arrive at 9:00 a.m. PAs and returning leaders come at noon!

July 14th
Camp setup and visitation day! Swing by and tour camp and see your unit from 1:00 p.m. to 3:00 p.m.

July 15th
First day of camp starts at 9:00 a.m.! See you there!
MAIL-IN REGISTRATIONS: 
(Only if you choose NOT to do online registration)
Please include Camper Registration, Health History, Medication Permission, Camper Release, and Behavioral Agreement forms.  Also include PA / LIT Application forms, if applicable.
Cancellation / Refund Policy

**Cancellations received before April 15, 2019 will have camp fees fully refunded.

**Cancellations received by June 1, 2019 will be refunded, minus a $35.00 cancellation fee. 

**After June 1, 2019 no refunds will be issued. 

**The $25 Girl Scout National Membership fee is 
non-refundable at any time.

Please add $25 National Girl Scout Membership fee for all girls 1st - 12th and adults if NOT currently a Registered Girl Scout.
Register by the grade camper will be in the Fall school year.
